A public intake form your prospects can actually use from their phone.

Drop the link on your website, your Google profile, or a QR code on your truck. Prospects fill it out, you get the request, and they get a text back in under a minute.

What this solves

You are losing leads at the front door. Calls go to voicemail. Texts pile up while you are on a roof. The Facebook DM you never saw turns into a one-star review three weeks later. Most small service businesses leak two or three leads a week this way, and they don't notice because there is no record of the ones that never came in.

The intake form is a single URL you can put anywhere. Your Google profile, your truck wrap, the bottom of an invoice, a Facebook post. When a homeowner fills it out, the request lands in your inbox with the photos and the service they asked about, and an auto reply hits their phone before they put it down. That is the difference between a lead and a "they never got back to me."

You do not need a website to use it. Some of our shops just paste the link in their Instagram bio and call it done.

How it works

  1. Get your link.

Every EosLog account comes with a public intake URL at eoslog.com/r/your-business-name. You can also embed the form on a website you already own with a one-line iframe.

  2. The prospect fills it out.

Name, phone, address, what they need, when they need it, photos. The form picks up the services you offer so they can choose from a real list, not a free-text field.

  3. You get the request.

It lands in your Requests inbox with everything attached. UTM tracked, so you know whether it came from your Google profile, a Facebook post, or the QR code on your truck.

  4. They get a text back.

A custom SMS auto-reply hits their phone in 60 seconds: "Thanks for your request. We got it and will be in touch shortly." You can edit the message. The text fires from your real business number, not a generic short code.

Common questions

Can I use this without a website?

Yes. Paste the URL in your Google Business Profile, your Instagram bio, your Facebook page, or print a QR code for your truck. You do not need a domain.

Does the auto-reply text come from my number?

Yes, once your SMS is activated. We provision a business number for you (A2P 10DLC registered with the carriers, takes a few days). After that, every auto-reply and every SMS in EosLog goes out from that number.

Can I customize the form fields?

You can choose which services appear, edit the SMS reply, and turn the price-range field on or off. Deeper field customization is on the roadmap.

What happens to spam submissions?

There's a per-IP rate limit and a length cap on every field. Most form-bot traffic never reaches your inbox. The rest you can mark as spam in one click.

Can a prospect attach photos?

Up to five, taken straight from their phone camera. The photos attach to the request and carry through to the quote and the job.
