Equipment condition report questions, answered
What is an equipment condition report?
A short form a technician fills in on arrival, before touching the equipment. It names the unit, records what was running, leaking, rusted, cracked or missing, notes the photos taken and writes down what the customer said was wrong. It is the starting point everything after the visit is compared with.
When should the technician fill it in?
Before work starts, with the customer present when possible. A condition noted at the start is a record of what you found. The same observation written after a problem shows up reads like an excuse.
What belongs on the form?
The date, site and unit (make, model, serial number and location), the symptom the customer described, what you saw on arrival, any damage or leaks already present, the photos you took, and the customer's signature next to yours. This form has a line for each.
Does a signed condition report protect me from a claim?
It is evidence of what you saw and showed the customer before starting, which is useful if a dispute comes up later. It is not a release of liability, and what it can do depends on your state and your contract. For anything beyond record keeping, check with a local attorney or your insurer.
Do I need an account to use this form?
No. Download the blank form and print it, or fill in the visit details online and download a PDF. Neither needs an account.
Keep the condition record with the job
In EosLog the completed form and its photos stay attached to the job and the customer, so what the unit looked like on arrival is one tap away when the call comes back.
