What’s in this template
Pest Control Service Agreement

- Fix the conditions Pest Control Company points out in a service report, such as open food storage, standing water, damaged screens or gaps in the building.
- Tell Pest Control Company about anyone in the home who is sensitive to treatments, and about any other pest control provider working at the property.
- Report any pest activity between visits within a reasonable time.
6. If a Covered Pest Returns
If the customer reports a covered pest between scheduled visits during the term of this agreement, Pest Control Company will return within: Return Visit Window, and re-treat at no extra charge. This does not apply to pests listed as not covered, to activity linked to a condition the customer was asked to fix and did not, or to damage that was there before service began. Treatment does not repair damage a pest has already done.

Pest control service agreement questions, answered
What should a pest control service agreement include?
At minimum: who the parties are and where the service happens, which pests are covered and which are not, which areas are treated, how many visits and when, the price and billing schedule, what the customer must do to prepare, what the company will do if a covered pest returns between visits, how the agreement renews, and how either side cancels. This template has a section for each.
Why list the pests that are not covered?
Because "general pest control" sounds like every pest to a customer. Termites and other wood-destroying insects, bed bugs, wildlife and stinging insect nests are often handled under a separate agreement or quoted per visit. Writing the excluded list into the agreement is what keeps a quarterly plan from turning into an unpriced promise.
Does this template meet my state's automatic renewal or pesticide rules?
This template does not claim to. Automatic renewal disclosures, limits on liability, cancellation rights and pesticide application record rules differ by state, and some states regulate structural pest control agreements specifically. Check with your state pesticide regulator and have a local attorney review the agreement before you use it.
Can I leave the guarantee out?
You can, but then the agreement says nothing about what happens when a pest comes back between visits, and that gap is where most callback arguments start. The form has a short re-treatment clause with blanks for the window and the number of free returns, so you set the limit yourself.
Do I need an account to use this form?
No. Download the blank agreement and print it, or fill in the header details online and download a PDF. Neither needs an account.
