Check this only if you're giving this form to a partnership, trust or estate that you own part of, and you have any foreign partners, owners or beneficiaries. Most people leave it unchecked.
Line 4: Exemptions. Most people leave this blank.
The codes apply only to certain entities, not individuals. They're listed in the IRS instructions for line 4.
Requester and account numbers (optional)
The business that asked for the form. Up to 3 lines.
Part I: Taxpayer identification number
9 digits. Hidden when you leave the box.
Your number is typed into the PDF inside this browser tab. EosLog never receives it, it isn't saved, and it's cleared when you leave the page.
Part II: Sign and date
Printed next to your signature, so an unsigned form is left undated.
Cross out item 2 (rare)
The form says to do this only if the IRS has told you that you're currently subject to backup withholding because you failed to report all interest and dividends on your tax return.

Questions
Why is my client asking me for a W-9?
Businesses that pay you use it to get your correct name and taxpayer identification number, so they can report what they paid you to the IRS on an information return such as a 1099. See About Form W-9 on IRS.gov.
Is it safe to fill out a W-9 online?
On this page, your number is put into the PDF inside your browser and never reaches EosLog. Take care when you send the finished form, because it shows your full tax ID. If your client offers a secure upload, use that instead of plain email.
Do I send the W-9 to the IRS?
No. It goes to whoever asked for it, not the IRS. The top of the form says so. This isn't tax advice.
Which tax classification do I pick on line 3a?
The one for the person or business on line 1. A business you own alone without an LLC or corporation is a sole proprietorship. An LLC with one owner that hasn't chosen to be taxed as a corporation is a "disregarded entity": it checks the box for its owner and puts the owner's name on line 1. Other LLCs check LLC and enter C, S or P. The IRS instructions for line 3a explain each one. This isn't tax advice.
Should I enter my SSN or an EIN?
Individuals generally use their SSN, or their ITIN if they don't have one. A sole proprietor with an EIN may enter either. A one-owner LLC that is a disregarded entity uses the owner's number, and an LLC taxed as a corporation or partnership uses its own EIN. See the IRS instructions for Part I.
Do I have to sign it?
For payments for your services, the IRS instructions for Part II say you must give your correct TIN but don't have to sign unless you have been notified that you have previously given an incorrect TIN. The person asking for the form may still ask you to sign, so this page lets you draw or type a signature.
